[ARCHIVED] GHS Chromebook, Textbook & Supplies Distribution

Hello Gardner High School Parents/Guardians & Students - 

In order to get chromebooks and needed supplies to our students for the 2020-2021 school year, we have scheduled distribution days where we will hand out chromebooks and supplies (books and other class supplies) to our families.  We will also need your assistance with completing the required beginning of the year paperwork.  We need to collect accurate contact information from all of our families and you will be asked to double check and update your student’s Verification Form (contact information, phone numbers, etc), update medical and medication forms and complete free/reduced lunch forms.  We will have all these forms available during these times, but if you are able you can also print the forms attached below, fill them out, and bring them with you. (These are PDFs and not editable online forms.) There will be stations set up around the school as you drive through.  You will remain in your cars.  Please pull up to the front of the school (Station 1). There will be staff there to guide you through this process.  If your student already has a reliable electronic device at home, you do not need to pick up a chromebook, but if possible we would still like for you to come by to complete the paperwork and also to pick up any other supplies your student might need.  Teachers will be sending out emails to parents and students if there are textbooks or supplies for you to pick up for your classes.  Also, if you have a chromebook or textbooks from the past school year to return, please bring those with you.  Thank you in advance for taking the time to come by the high school during any of the scheduled times below, as this process is critical for the success of your student’s school year. 

Schedule: 

Tuesday, September 8th from 3:00 pm - 7:00 pm

Wednesday, September 9th from 3:00 pm - 7:00 pm

Friday, September 11th from 9:00 am - 12:00 pm

You may come on any of these dates during these scheduled times.


Please bring the following items in order to complete documents: 

  • Writing utensil

  • Insurance Card (for Medical Forms)

  • Emergency contact information (for Verification & Medical Forms)

  • Medical information, list of medications  (for Medical Form and Medication Approval Form if needed)

  • Contact information (email/phone) from parents/guardian and student (for Verification & Medical Forms)  

  • Transitional assistance number, weekly, bi-weekly or monthly income amounts (for Free/Reduced Form)
