How to Obtain a Death Certificate
The City Clerk’s Office is responsible for original records of all deaths that have occurred in Gardner, including Heywood Hospital. All death certificates are public records. A certified copy of a death certificate may be obtained in person or by mail. The fee is $10.00 per copy.
To pick up certificates in person: Walk-in requests are accepted and you may wait while the certificates are prepared. Cash and checks are accepted forms of payment. Our office is located at 95 Pleasant Street, Room 121, Gardner, MA 01440.
To receive certificates by mail: Please fill out the attached Application for Vital Record and return with a check or money order payable to the "City of Gardner".
